This questionnaire is designed to gather the basic information that The Franchise Firm LLP (the "Firm", "We" or "Us") will need in order to file for your trademark application. Your completion of this form will assist us in timely applying for your trademark application either under Section 1(a) as being used in commerce or Section 1(b) as an intent-to-use application. Please email us with any questions. Note that after we apply for your trademark, you will get notification by email from the USPTO of the application. You will also receive mailings and emails related to the application requesting money. Do not pay any third party any money related to your trademark as there are spam companies that solicit funds from unsuspecting applicants. We will notify you when the USPTO examines your application which usually occurs within 3-4 months. The total time for a trademark application currently is approximately 12 to 14 months unless there is a refusal or opposition filed which could extend that time further or cause the USPTO to reject the application for reasons that will be explained by counsel. This questionnaire will be reviewed by the Firm and does not, by itself, constitute an attorney-client relationship. The Firm only serves as counsel to clients who have signed engagement letters.
